Patient Update Request

Follow Up on the Animal You Brought to the Wildlife Center

Please fill out the following form with as much information as possible so that we can easily find the electronic medical chart for the patient you brought in. We will respond to patient update requests via email within 14 days. We ask that you do not submit more than one status request per month. Care for our patients comes first, and we must leave our wildlife hotline open for emergency calls. Replies will come from patientupdate@westshorewildlife.org (add this to your email contacts to help ensure updates do not go to your spam folder).
